We need you to sign and return the ‘Section F’ declaration we have sent you to confirm the reasons for your Payment Protection Insurance (PPI) complaint, as outlined in your PPI consumer questionnaire.

We will also need the Financial Ombudsman Service (FOS) declarations, Letters of Authority and the FSCS questionnaire if enclosed.
