FSCS Awaiting Questionnaire

Further to our conversation, we will now complete your FSCS questionnaire as much as possible and will send it to you in the post for review.

When it arrives, please:

• Check all the information is correct
• Fill in any blank parts of the questionnaire
• Provide the forms of identification noted in section E
• Sign and date the documents
• Return them in the prepaid envelope

We need this in order to complete our application. We have a very high success rate of cases through the FSCS so please don’t delay returning your questionnaire once it has arrived.
