FSCS Questionnaire Sent To You

Your FSCS questionnaire is in the post for you to review.

When it arrives, please:

• Complete the highlighted sections and check the information completed on your behalf for accuracy
• Sign sections G & H
• Fill in your bank details
• Complete and sign the Letters of Authority at the back
• Provide proof of identity (3 pieces for each of you, as per the list in section E)
• Return all documents in the prepaid envelope provided

Our success rate with cases that pass through the FSCS is almost 85%* so please don’t delay in returning your paperwork.
